Use of the AQoL instruments is free to all users, subject to copyright restrictions.

Steps

1. Registration Form - Users please register for each study using the online registration form hosted at the Centre for Health Economics website.

2. Questionnaires - Select the appropriate instrument(s) to use for your study

• Option 1: Download the questionnaire and demographic questions (if desired), run your own survey. This is the usual way of using the instruments. You can collect your data, download the scoring algorithm and calculate your scores.
• Option 2: Use the Monash University ONLINE system - We can calculate your scores. We can send your respondents a link to our instruments which are all available to complete online. We will send you the data collected.

3. Scoring Algorithm

• Option 1: Download the algorithm and instructions to calculate your scores.
• Option 2: We can calculate your scores.
