Use only the AQoL-4D data collection questionnaire.  This scoring algorithm is NOT for calculating utility scores, it is unweighted and uses the raw coded answers to score the questionnaire.

The items should be labelled aqol1, aqol2, aqol3 ... aqol12.


The item responses should be entered as 1, 2, 3 etc according to the presentation order in the questionnaire. Each item has 4 response levels.

*Missing Values: Note that missing data are represented by a blank or dot and are handled by imputing values within each dimension. Dimensions with 3-4 items will allow for 1 missing value to be imputed, dimensions with 7-8 items will allow for 2 missing values. However, if more item responses in the dimensions are missing the observations will be dropped and there will not be a dimension score or an instrument score for the individual.

 

Step 1:  Right click on this AQoL-4D SPSS unweighted syntax file and choose 'Save Target As...’ or 'Save Link As...' and save the file to your preferred location. 


Step 2: Open your SPSS data file containing the AQoL data.


If your data file is in Excel, Access or STATA format, you need to convert it into SPSS by opening SPSS and choosing File -> Open -> Data and then selecting your data file.


Step 3:  Next select  File -> Open -> Syntax and open the file you saved in Step 1.

 

Step 4:  Select Run -> All

This will produce an output file containing the results of the algorithm and some descriptive statistics. In your data file there will be new variables and the final column will be the total AQoL score.
